Harris-Galante Acetabular Replacement in Osteonecrosis of the Femoral Head

Abstract
Purpose: This study was undertaken in order to evaluate the results of uncemented total hip arthroplasty using Harris-Galante cup in patients with avascular necrosis of the femoral head(Ficat and Arlet stage ¥² and ¥³).

Materials and Methods: Between February 1989 and July 1995 sixty total hip arthroplasty procedures were performed in patients with avascular necrosis of the femoral head using a harris-Galante cup. The mean follow-up period was 7.6 years and the mean age of the patients was 48.3 years old. We evaluated both clinical and radiographic parameters.

Results: The mean preoperative hip score was 52.8 points, which improved to 92.5 points at 1 year postoperatively and 93.2 points at 3 years postoperatively. Subsequently, the hip score decreased and was 90.5 at 5 years postoperatively and 86.7 at the final follow up. At final follow-up, groin pain was registered in twelve hips(20%) and thigh pain occurred in fourteen hips(23.3%). The frequency of osteolysis aroung the cup was fourteen (23.3%). Regarding the radiographic instability of the acetabular cup, forty eight hips(80%) were stable and twelve hips(20%) were probably unstable. The average linear wear rate of the polyethylene was 0.25 §®/yr. There were four cases of poyethylene liner dissociation, and ten cases of massive osteolysis.

Conclusion: Clinical and radiographic results using Harris-Galante cup combined with uncemented stems in osteonccrosis of the femoral head were favorable, althought mechanical problems occurred due to the Harris-Galante cup itself.
